Printable Forms for All Medications and Medical Conditions

Note that some of the forms will require a doctor's signature as well as a parent's signature.

All forms valid for the current school year and will not be carried over to the following year!

*Incomplete / Incorrect forms will result in the Nurse being unable to follow the proper orders in caring for your student and/or administering the medication. This protects not only your child but also the Nurse's license. Forms and/or accompanying medication will NOT be accepted if not completed in full. Thanks for understanding!

Required Paperwork for ALL Medications {OTC and Rx}

ALL medication must be kept in clinic. Please do not send your child to school with medicine to keep in their pocket or backpack--it is not allowed. If you would like to keep any medication at school for your child, please print and complete the "Medication Administration Request Form" below and bring it to the clinic along with the medication. Also note that if your child will need more than 10 doses of any over-the-counter medication {Tylenol, Tums, etc.} I will need a note from their doctor indicating the need for multiple doses while at school. {For example "Child suffers from migraines. Please administer Jr. Strength Tylenol according to package directions as needed for the 2020-2021 school year."}

All medications must be in their original container, children's strength and not expired.

All prescription medications must have the prescription label attached. Please see "FISD Medication Policy" page of this website for more information on what medications are allowed at school. Feel free to contact me any time you have questions regarding this policy. I know it's confusing. I'm here to help! I would much rather you ask in advance than bring medicine to school and find out that you'll need a doctor's note before we can use the medication at school. However, if is it not on the approved list, the rule of thumb is to provide a doctor's note or arrange a time to administer the medication at home outside of school hours instead.

FISD policy prohibits me from administering any medication unless I have a completed {by you, and if necessary your child's doctor} form on file for the current school year for the medication. A handwritten note from home, email, text, or phone call is not acceptable.

Please note that I cannot keep any medication in the clinic over the summer. All medication must be picked up by parents no later than the last day of school. FISD does not allow me to send medication home with the students. All medication not picked up by that date will be disposed of in accordance with FISD policy without exception.

Once all of these items are completed by a parent/guardian and your doctor, please bring them to the school Please inform the front office to notify you are at school to drop-off medicine. Medication Drop-Off

Nurse Salgado is available during school hours and on staff days for medication drop off/pick up. However, please note medicine cannot be dropped off during morning arrival time.

*Please be advised that in order to keep medication at school, you must also complete these items:

1. Make sure the medication can be given at school. We only give medications that cannot possibly be given outside of school hours by a parent. For example, a prescription dose written to be administered at 12:30PM daily would need to be kept at school and given by the RN. Morning and afternoon medications are to be given by a parent before/after school. A parent should make all efforts to have medications administered outside of school hours when possible.

2. Any as-needed, over-the counter medications will not be kept at school this year except under special circumstance and doctor-orders.

3. Asthma – only inhalers with spacers can be kept at school; no nebulizer treatments.

4. Students may NEVER carry any medication to/from school; this includes cough drops, topical prescription creams, vitamins, essential oils, etc.

5. It is okay if your student carries Aquaphor, Vaseline, Chapstick, or other non-medicated lotion.

5. Before coming to the school please ensure the required forms are filled-out by your doctor AND by yourself (both need to sign). Also ensure the medication is not expired and the label matches the doctor-ordered information on the forms. See the links below for forms required. All medications must be turned-in to the nurse by a parent/guardian with the completed/signed appropriate forms.

SPECIAL PROCEDURES

Guidelines:

• Physician’s order and written permission from a parent/guardian must be obtained before a special procedure can be done on a student. Permission will only be valid for the current school year and will not be carried over to the following year.

• Parent must provide and maintain all equipment and supplies necessary for procedure.

• Any change in the procedure is the responsibility of the parent to notify the school nurse.

• Parent must demonstrate the procedure initially to the school nurse to maintain consistency between home and school.

A Special Procedures: Physicians Authorization For Special Health Care Form may be requested from your School Nurse. Additional forms may also be required such as medication forms. Please refer to the Medications section of this site for more information.

Special Procedures examples include: Tube Feedings, Urinary Caths, Oxygen Therapy, Suctioning, Tracheostomy Care, Ostomy Care, Any Special Assistance varying from routine, Routine VS checks, etc.


HERBAL AND DIETARY SUPPLEMENTS

In order for students to receive Herbal & Dietary Supplements while at school, all of the criteria below must be met. Please contact the Special Ed dept. at your campus if a parent requests that an Herbal and/or Dietary supplement be given. The Special Ed dept. will determine if the student meets the criteria below. In the event a student does meet criteria, the Special Ed dept. will have an ARD & the Physician’s Order for Herbal & Dietary Supplement form will be given to the parent for the doctor and parent to complete. FFAC (local) policy states Herbal substances & dietary supplements provided by the parent & only if:

• The child’s personal physician has ordered that such product(s) be administered;

• The order specifies that the product(s) must be administered during school

hours; and

• The child’s individualized education program (IEP) or 504 Plan reflects that the product(s) are necessary for the provision of a free, appropriate public education for a student with a disability.
