FRHSD Athletic Participation Information & Required Paperwork

Required Paperwork for Athletic Participation: 

All athletes must submit required forms each season that they hope to participate in, according to the process detailed below:

A. A valid Pre-Participation Physical Evaluation Packet dated within one year of the season’s start date, that includes a physical evaluation completed by a licensed physician.

1. To determine if the date of your last physical is valid: Review the sports physical exam date found on the “Summary” tab in the Genesis Parent Portal OR Students can check with the Health Office at school.

1. If a valid physical IS on file in the health office (within one year of the season’s start date): 

• Go to part B, you do not need to complete the Preparticipation Physical Evaluation Packet for this season

2. If a valid physical is NOT on file in the health office (NOT within one year of the season’s start date): This Pre-Participation Physical Evaluation Packet is required for this season. Print and complete ALL 5 PAGES with the same date and submit by the deadlines above to the Health or Main Office.

2. The Pre-Participation Physical Evaluation Packet includes the following forms:

2. 1. Pre-Participation Physical Evaluation History Form must be signed by parent and student. Please explain all "yes" answers; please include dates, length of symptoms, and treatment. This form is kept by the healthcare provider completing the physical exam. 

   2. Athlete with Special Needs Supplemental Form only needs to be filled out if applicable.  This form is kept by the healthcare provider completing the physical exam. 

   3. Physical Examination Form: The form must be complete including height, weight, vision, pulse, and blood pressure. All abnormal findings must be explained. The date of the exam must be written by the licensed physician and the form must be signed by the licensed physician as well. If you wear glasses or contacts, remember to bring them with you to your physical examination. This form is kept by the healthcare provider completing the physical exam. 

   4. Medical Eligibility Form: The form must be complete and signed by the licensed physician. In addition, your physician must complete and sign off on the Cardiac Assessment Professional Development Module on the form.  This form must be submitted to the health office or main office by the deadline listed above.

B. The following forms must be completed online in the Genesis Parent Portal, accessed in the “Forms” tab: 

1. Activity / Athletic Eligibility Form: Must be completed EACH SEASON by the submission deadline for students participating in multiple sport's seasons.

2. Health History Update Form: Must be completed EACH SEASON (after the initial season covered by the original physical form) by the submission deadline for students participating in multiple sport's seasons. Please explain all “yes” answers thoroughly.

3. Athletic Emergency Information: Must be completed ONCE each academic year.

4. Sports-Related Concussion and Head Injury Fact Sheet: Must be completed ONCE each academic year.

5. The Sudden Cardiac Death in Young Athletes Information Sheet: Must be completed ONCE each academic year.

6. NJSIAA Steroid Testing Policy: Must be completed ONCE each academic year.

7. Use and Misuse of Opioid Drugs: Must be completed ONCE each academic year AND Athletes vs. Opioids video must be viewed each season.

8. Parental Transportation Form‐ (must be completed once each academic year).   This form is not on the portal
