All student athletes must have yearly athletic physical completed and turned into the FMS main office in paper form prior to trying out or competing for all Fremont Middle School athletics.  The OHSAA athletic physical will expire 13 months after the last time a physical exam was performed by a physician.  Listed below is a link to the OHSAA website to download and print a physical form. Click on the current school year PPE Forms. Please make sure to turn in all the pages listed in the document to the Fremont Middle School office or to the athletes coach.
