In Kind gift Receipt
All donations to the Center are in-tilted to a gift and kind receipt. Person donating items must fill out Gift and Kind paperwork. Person donating is also responsible for coming up with total dollar amount donated for this paperwork. Once completed please mail paperwork to address listed.
