What is Person Centered Planning? What Is Person-Centered Planning?
Person-centered planning is an ongoing individual planning process that is designed to capture your dreams and desires and translate them into a plan of action. Person-centered planning is a way to listen and take direction from you and the people who know you best. It focuses on your preferences, strengths and talents rather than your limitations. Person-centered planning organizes and uses natural supports like family, friends and acquaintances and formal community supports and services to help you achieve the things that are important to you. Examples may be MAPS & Charting the Lifecourse.
Supported Decision Making What is supported decision-making?
Supported Decision Making (SDM) is an alternative to conservatorship or guardianship that offers individuals with disabilities more independence. People can use SDM to make their own decisions instead of having someone else make decisions for them. When you use SDM, you can have more control over your life – more self-determination. When people with disabilities have more self-determination, they can have better lives. They are more likely to work, live independently, and be safer. An SDM plan says when you want help, how you want to be helped, and who will help you. The people in your life who support you make an agreement to follow your plan.
Conservatorship What is conservatorship?
Conservatorship is a court-ordered mechanism to ensure the well-being of adults who lack the capacity to manage their own affairs, whether it's their finances or personal care.
Guardianship What is guardianship?
Legal guardianship is granted for some adults with intellectual disability to help manage care, housing, supervision, and other important parts of life. The term “respondent” is someone for whom a guardianship petition has been filed. If the Probate Court grants guardianship, the respondent is then referred to as a “protected person.”
Power of Attorney in CT What is a Power of Attorney?
A Power of Attorney is a legal document you use to allow another person to act for you. You create a legal relationship in which you are the principal and the person you appoint is the agent. A Power of Attorney specifies the powers you give to your agent. The powers can be limited or broad. For example, if you are selling your house, but unable to attend the closing, you can give someone the power just to sign the deed in your absence. Most durable powers of attorney, however, give your agent the power to do almost anything you could do.
