Transition planning starts early and is an ongoing process, helping students build post-school goals and successfully move from secondary education to adult life. The FPS team is dedicated to supporting students and families throughout each stage of the transition. Guided by Connecticut’s Core Transition Skills Standards, we collaborate with students, families, and educators to develop individualized plans that foster independence, career readiness, and community engagement.
