Matriculated Student Status Form

• Once you have completed your coursework, you MUST submit this form EACH semester that you register to remain eligible as a "full-time student" for your funding or to be eligible for student loans.

• Use this link to find the application form for the Inter-University Doctoral Consortium. **Note, only Second-Year Phd Students are eligible for the IUDC. 

• Use this link to find the application for the New York Theological Group Consortium. **The NYTG is open to MA/MTS students as well as first -and second-year PhD students.

Graduate Tutorial Form 

• If you would like to sign up for a tutorial that you have arranged with one of your professors, you will need to fill out this form. 

• Return the form to the Theology Department secretary, who will record the information and submit the form to GSAS.

GSAS 0-Credit Language Study Application

• Fill out this application if you would like to take undergraduate language courses for your graduate work (e.g., Greek, Latin, Arabic).

Information regarding the auditing of a class 

• Information about auditing a class can be found in section 5.3.6. of the Bulletin.

Request Form for an Incomplete

• If you need to request an incomplete in one of your courses, fill out this form and secure the signatures of your course instructor, the Chair/DGS, and the GSAS Dean. Instructions are included on the form; make sure to adhere to the timeline provided for submission.

Course Withdrawal Late Registration Form

• Submit this form if you need to add or drop a class after the add/drop deadline.

Request for a Medical Leave of Absence

• If you feel the need to take a Medical Leave of Absence, please review carefully the policy, and the steps that must be taken to secure the leave and to fulfill the requirements for re-entry upon your return. The GSAS policies for a Medicial Leave of Absence can be found HERE.

Withdrawal from University Form
