According to Ohio Revised Code 3717-1-02.4
A) Person in charge - assignment of responsibility.
(1) The license holder shall be the person responsible for the food service operation or retail food establishment. The license holder may be the person in charge or shall designate a person or persons in charge and shall ensure that a person in charge with applicable knowledge is present at the food service operation or retail food establishment during all hours of operation. This paragraph does not apply to a micro market as defined in Chapter 3717-1 of the Administrative Code.
(2) One year after the effective date of this rule, at least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
(3) Temporary, mobile, vending and risk level I and risk level II food service operations or retail food establishments are exempt from paragraph (A)(2) of this rule.

Level I training is a 2-4 hour Food Safety Basics class and has been referred to as Person -In-Charge Training in the past
Level II training is required to be a minimum of 15 hours and an exam is administered to receive certification
The course shall be taught in a minimum of fifteen verified contact hours, excluding examination, to cover the goals and objectives as outlined in the provisions of this program. Instruction time and techniques may be modified when a review of the participants reveals learning disabilities, language barriers or other factors, which may impact learning.

Individual Certification In order to become “Certified in Food Protection” an individual must meet the following:
1. Successfully complete a Certification in Food Protection course offered by a course provider that has been approved by the Ohio Department of Health; and
2. Successfully pass an acceptable Food Protection Certification Program Examination; or
3. The conditions of reciprocity as stated in Section V.
SECTION V RECIPROCITY
An application to become certified by the Ohio Department of Health through reciprocity shall include proof from an individual that they have successfully completed a substantially similar certification program as outlined in the provisions of this program.

No, they no longer automatically send a certificate
You can get this for yourself by
Calling ServSafe and requesting one - the cost is currently $10.80
You can go to www.servsafe.com and create an account and then call ServSafe to have them link your test scores to the account you created. Once it is linked, you will be able to print your certificate from ServSafe.
Currently, Ohio requires you to successfully pass a 15 hour approved course that has an examination to receive an Ohio Certificate
A passing score on your ServSafe exam allows you to receive an Ohio Certificate
The Ohio Certificate does not currently have an expiration
You MUST have an OHIO CERTIFICATE when the health department comes to do an inspection
You will receive the Ohio Certificate from FND Consulting Services via email
ServSafe is an approved course to receive your Ohio Certification once you have successfully passed the ServSafe exam
The ServSafe Certificate is only good for 5 years
You do not have to have this certificate for the health department as there are other Ohio approved courses
It traditionally takes about 2 weeks to receive the score for the ServSafe Exam
Once the score is received, FND Consulting Services will forward the passing grades to the Ohio Department of Health
Once the Ohio Department of Health returns the Certificates via email, the Ohio Certificate will then be emailed to you
You may take the exam twice within a 30 day period after the first failed attempt
You may register for any of the dates listed on the Upcoming Training tab, click on the date, and register for the TEST ONLY
You will need to only come the second day of the training after lunch around 12:30pm to review and take the exam
You must wait 60 days after your second or third failed attempt
You may only take the test 4 times in a 12 month period
