Compliance Forms

Sign into the PARENT FOCUS PORTAL to complete the compliance forms.

Username will be the email address used when creating the parent portal account. 

Please click “Forgot Password” if parent does not remember and needs to reset the password.  

These forms will need to be completed for each student enrolled at FLVS.
