CHW Awards


2024 Community Health Worker Awards 


The Florida Community Health Worker Coalition would like to honor outstanding individuals that are identified by their peers, community, supervisors as having consistently demonstrated care, excellence and dedication in the role of Community Health Worker or have advocated for or advanced the CHW workforce. The award will be presented at the 2024 Florida Community Health Worker Summit Award Luncheon - "Honoring CHW from Across Florida". 


AWARD CRITERIA

Nominations made for individuals who are exemplary stakeholders or Community Health Workers who are currently working in the role for which they are nominated. Award recipients are determined solely on the content of the written nomination which will be limited to 500 words, and selection of the review committee. 

Contact information for both the nominator and the nominee is required. The number of nominations as individuals received will not affect the voting process or outcome.  


AWARD CATEGORIES

CHW Difference Maker Service Awards:   This category will recognize up to four CHWs for their leadership, teamwork, and/or dedication and service in the community from the health and social service sectors. Nominees should highlight the individual's work within a community or with individual clients/patients, leadership or mentoring roles or how that individual has worked to promote the CHW profession. Two urban CHWs and two rural CHWs will recognized in this category 


Community Health Worker Champion Award: this category will recognize individuals, community partners/stakeholders, or organizations within the health and social service sectors for their efforts to champion for CHWs through advocacy, promotion or workforce advancement,  (e.g. leadership, or employment of CHWs). 


Community Health Worker of the Year:  this category will recognizes a CHW who has gone beyond to serve and who has worked diligently and consistently with, and advocates for their profession and behalf of the clients/patients in their community. Must be nominated by a team of two or more co-workers or supporters. 


Award recipients will be announced at the the 2024 Florida Community Health Worker Summit - Award Luncheon. Nominations are closed.
