5W+H questions can be formed using this formula : QUASM? ( Qu estion word + A uxiliary verb + S ubject + M ain verb ? )
What is the auxiliary verb? An auxiliary verb helps the main (full) verb and is also called a "helping verb." With auxiliary verbs, you can write sentences in different tenses, moods, or voices. Auxiliary verbs are: be, do, have, will, shall, would, should, can, could, may, might, must, ought, etc.
Take a look the following examples:

What are you doing?
Where
Where do you live?
When
When did you get home?
Who
Who were you talking with?
Why
Why do you love me?
How
How could you leave me?
(some time you need to add an adverb at the end of your sentence)
