The expulsion history form will need to be notarized, which can be done during your enrollment appointment.


Enrollment Requirements

You MUST have ALL of the requirements fulfilled prior to making an appointment to enroll.

 

 A completed immunization record signed by a physician/health department official or a school immunization record. Required immunizations include :( Printable Version)

· Diphtheria, Tetanus, & Pertussis (DTaP, DTP, or Tdap): A minimum of 4 doses with one of the doses on or after the fourth birthday. Effective July 1, 2006, a booster dose of Tdap vaccine is required for       all children entering the 6th grade, if at least five years have passed since the last dose of tetanus-containing vaccine.

· Polio Vaccine: A minimum of 4 doses of polio vaccine. One dose must be administered on or after the fourth birthday.

· Varicella (Chickenpox) Vaccine: All susceptible children born on and after January 1, 1997, shall be required to have one dose of chickenpox vaccine administered at age 12 months or older. Effective March 3, 2010, a second dose must be administered prior to entering kindergarten or at appropriate medical interval between dose 1 and dose

· Measles: Two (2) doses of live virus measles (rubeola) vaccine, one dose given 12 months of age or older and a second dose administered prior to entering Kindergarten or 1st grade, whichever occurs first (effect July 1, 1991).

· Rubella: One (1) dose of rubella vaccine received at 12 months of age or older.

· Mumps: One (1) dose of mumps vaccine received at 12 months of age or older.

· Hepatitis-B Series: Three (3) doses.

· Proof of negative tuberculin screening done within 30 days prior to registering for all students who resided outside of the USA for three consecutive months within the past five years.


NOTE: Effective July 1, 2021 two properly spaced doses of meningococcal conjugate vaccine ( MenACWY).  The first dose shall be administered prior to 7th grade.  The second dose shall be administered prior to the 12th grade.  Students in grades 8 and above ( school year 2021)  will not need to go back and receive the 7th grade MenACWY dose, and they will pick up with the 12th grade MenACWY dose.  Rising 12th grade students will be required to have 1 dose of the MenACWY prior to school entry.
