Use this form to register your student each year.

Use this form to give permission for your child to participate in a class field trip.

Medication Dispersement Form

This form is needed to give permission for a school staff member to administer over the counter medicine to your student.

Social Media Release Form

This form is needed to be completed for every student, so we know if we have permission to use photos/name of your student in newspaper articles or on social media (i.e. Facebook postings, etc).

Permission to Leave for Lunch.pdf

Parent Consent to Leave for Lunch Form

This form needs to be completed for any Junior or Senior grade level student that would like the priviledge to leave campus for lunch.

** Student's must get back in time for the following class
in order to retain this priviledge.
