Required if the Delegate will take medication:

      Medication Authorization Form  (one per medication: prescription and/or over-the-counter)

For EACH over-the-counter (OTC) medication and/or prescription medication that does not have the pharmacy label, you need this form:

      Physician’s Order  (one per medication: prescription and/or over-the-counter)

If your child is allowed to self-administer (i.e. Inhaler), you need this form:

      Self-Medication Authorization    (Español )  (one per medication: prescription and/or over-the-counter)
