Please complete each of the forms below. For your ease, all forms except the medical forms are digital.

The medical form and physical form must be hard copy since they must either be notarized or completed by a doctor.

Copies of these medical forms are available in PDF below or in paper copies in the band room.

1st Payment - Due May 15

2nd Payment - Due June 15

3rd Payment - Due July 21

TCHS Band Medical Form

This form must be completed by a parent or legal guardian. It will need to be notarized. We have band parents who can notarize it for you.

Physical Form

Part of this form must be completed by a doctor. Take this form with you to your child's next check up to have them complete it. This form can also be used for any other sport your child will participate in. You can give us a copy and keep the original for other sports.
