Referrals
Weekly Behavior Review: Each week the team tracks the number of consecutive weeks a student has been on the weekly review. The number of consecutive weeks on the list determines what steps the team may take.
Week 1: Monitor Progress
Week 2: Teacher referrals are available on JME Staff Google Classroom and are reviewed weekly.
Week 3: Observation of student in the classroom
Week 4: Student Conference to determine the possible cause of behavior
Week 5: Systematic Problem Solving (SPS) Meeting with teacher to develop plan
Week 6+: Monitor plan and revise as needed
2. Teacher Referrals: There are four forms for referrals. Teachers can fill this out, and then they will be reviewed by the Student Support Team weekly.
3. Parent Referrals: Parents can complete this request for Counseling services or if they just have questions or concerns.
4. Referral Procedures: Once a referral is received using one of the three methods above SST will complete the following:
Discuss options for services
Identify Case Manager
Case Manager completes the following as needed
Behavior Observation
Parent Conference
Case Manager meets with MTSS Lead and Teacher to discuss the following
Current Data
Behavior Intervention Plan
Chart as needed
Case Manager and Teacher work collaboratively to support the student
