Want to Get Your Driver’s Permit?
You will need a No Pass No Drive School Compliance Verification Form. Students must be 16 years of age to request a form and request the form 48 hours in advance. Also, students must meet the No Pass No Drive criteria according to the KRS 159.051 law (see below).
Each school handles forms for their own students. This information is for Henry Clay students only.
Use this LINK to request a School Compliance Verification Form. You must do this 48 hours prior to needing your document.
Ms. Daugherty, Student Support Administrative Assistant, will check attendance and grades to verify the student is eligible for the compliance form.
You will be notified if the student does not have a social security number on file. Students must have this in order to receive a verification form.
Ms. Daugherty will prepare the form and notify the student and/or parent when it is ready for pickup.
Make an appointment with the DMV. The School Compliance Verification form is good for 60 days after it is issued by the school.
If you have any questions, please contact Kim Daugherty at kimberly.daugherty@fayette.kyschools.us.

To be eligible for a permit, the KRS 159.051 law states:
Students must pass at least 66% of their classes the semester prior to applying for a permit and each semester after obtaining a permit or driver's license. This means passing 4 out of 6 classes or 5 out of 7 classes or 6 out of 8 classes depending on how many classes are on the student's schedule.
Students must NOT have more than 9 unexcused absences in the previous semester. This can be calculated using partial days of unexcused tardies to school or dismissals from school and unexcused absences from individual classes such as skipping class.
