A sports physical is REQUIRED before any student can particpate in any athletics of any kind (open gym, conditioning or tryouts). Before submitting your athlete's sports physical, make sure that you have the correct KHSAA sports physical as pictured. If you are missing any of the pages, click on the images to download.
