This manual is the Student's Guide to the Fieldwork Program in the Faulkner University Occupational Therapy Program. It includes information for both Level I and II Fieldwork and the students are given this manual prior to starting Fieldwork I rotations.
Students are responsible for collaborating with educators via forms and other documentation to ensure they are meeting the fieldwork standards as determined by ACOTE. Please refer to the Fieldwork Manual for specific requirements.
Syllabi for Fieldwork IA, IB, and IC Rotations are important documents for both the student and the educator(s). Each syllabus includes objectives needed to successfully complete the fieldwork rotation.
Fieldwork IA Syllabus Spring 2025
Students should document daily hours worked on the Hour Log Form for FW I. These forms should be signed/initialed by the fieldwork educator.
Syllabi for Level IIA and IIB Rotations are important documents for both the students and the educator(s). Each syllabus includes objectives needed to successfully complete the fieldwork rotation.
FW II Students and Educators should participate in scheduled, weekly meetings. FW II Students will complete the Weekly Meeting Report about the meeting and have it signed/initialed by the fieldwork educator.
