Read through the Fieldwork Manual.
Complete the Educator Intent to Participate Form.
Complete Fieldwork Site Data Form (new sites only)
Complete the Site Specific Objectives Form (FW II only)
Meet with student weekly and sign record of weekly conference and/or Fieldwork II weekly meeting report.
Sign and/or initial hour log (FW I only).
Provide an appropriate amount of supervision for the specific student throughout the rotation.
Complete the Midterm FWPE (FW II only).
Complete the Level I Fieldwork Evaluations or Level II Final FWPE at the end of the rotation.
Evaluation forms are vital to the success of the program and are required for both Level I and II Fieldworks. Level I Fieldwork evaluations are completed at the end of the rotation. Level II Fieldwork evaluations are completed at the midpoint and at the end of the rotation. Please make sure you are completing the correct evaluation by the deadlines given and are able to use the FWPE-Formstack portal. Your feedback on students is important to their success now and in the future as they progress through the program.
All completed evaluations for Level I Fieldwork should be sent to abrenneman@faulkner.edu in a timely manner so that grades can be processed and any deficiencies can be identified. Fieldwork II Evaluations will be completed online and automatically update using the FWPE-Formstack Portal.
This manual is the Student's Guide to the Fieldwork Program in the Faulkner University Occupational Therapy Program. It includes information for both Level I and II Fieldwork and the students are given this manual prior to starting Fieldwork I rotations.
Students are responsible for collaborating with educators via forms and other documentation to ensure they are meeting the fieldwork standards as determined by ACOTE. Please refer to the Fieldwork Manual for specific requirements.
These forms should be signed/initialed by the fieldwork educator. Weekly Hour Logs and Meeting Reports will be scanned in by student and submitted on Canvas.
FW II Students and Educators should participate in scheduled, weekly meetings. The meetings should be memorialized by the student on the Weekly Meeting Report and signed/initialed by the fieldwork educator.
