Nomination Form for Educators and Alumni Achievements

Note: Nominees must have graduated or attended two or more years at FHS from 1986 - 2010.

Achievement Nomination

Requirements for individuals to be considered for recognition of achievements after high school: achieved recognition in chosen profession or career; made outstanding and notable contributions to society through charitable or volunteer work either individually or within an organization. When making nominations, include any awards or honors the nominee has received after graduation.

Education Nominations

Educators nominated must have been a teacher or in an Administrative position in Farmington R-7 School District for five (5) or more years. The individual need not have been an alumnus or even a student in Farmington Schools to qualify for nomination. (Coaches are not eligible in this category.)
