**IMPORTANT **

Prior to Entering 7th grade, all students are required to provide to the nurse a copy of an updated physical   along with proof of the following 2 vaccines:

    

   Tdap

1 dose; and history of DTaP primary series or age appropriate catch-up vaccination. Tdap given at ≥7 years may be counted, but a dose at age 11-12 is recommended if Tdap was given earlier as part of a catch-up schedule. Td or Tdap should be given if it has been ≥10 years since last Tdap.



   MenACWY (Meningococcal Vaccine)

   1 dose MenACWY (formerly MCV4) required. Meningococcal B vaccine is not required and does not meet this requirement.



**** PLEASE SEND A COPY OF YOUR STUDENT'S PHYSICAL EXAM AND IMMUNIZATION INFORMATION IN TO THE NURSE WHENEVER YOUR CHILD SEES THE DOCTOR FOR A PHYSICAL , NO MATTER WHAT GRADE YOUR CHILD IS IN. THAT WAY, IT IS EASY TO UPDATE YOUR CHILD'S HEALTH INFO, ESPECIALLY IF YOUR CHILD PLANS TO PARTICIPATE IN ANY SPORT WHILE IN 6TH, 7TH, OR 8TH GRADE.

SBIRT: Screening, Brief Intervention and Referral to Treatment is done in grades 7 and 10. Student screening sessions will be brief and conducted confidentially in a private, one-on-one session. Further information about this screening will be sent home prior to the screening date. 


Parents/guardians will be notified if a referral for follow-up evaluation is required.
