School-Based Health Program

Fairfield City Schools has partnered with Primary Health Solutions to provide School-Based Health Care to students in our district.  Enrollment in the program provides families with healthcare options including well visits, sick visits, vision services, and dental care.  We provide transportation to and from the clinic during school hours for your convenience.  For more information or enrollment paperwork, click the links below or contact me in the office.
