2. Once you are done, another link will be given for you to download and print the application form. (Note: The form must be printed--LONG SIZE BOND PAPER.)

3. Fill-up the necessary details in the form and DO NOT FORGET TO AFFIX YOUR SIGNATURE.

4. Provide 2 copies of your application form.

• For graduating students that live outside Tacloban City and will send their form through a courier service. Please write the following details below:

Name of Receiver: CAAD (College of Architecture and Allied Discipline) c/o Ar. Hervy G. Catilogo

• Graduating students that live in or near Tacloban City, you may drop your application form in the designated drop box at the guard house. Just inform the assigned guard that your document is for CAAD.

5. Your forms must be submitted inside a LONG BROWN ENVELOPE. Write "APPLICATION FOR GRADUATION" at the upper left back portion of the envelope along with your full name (Surname, First Name, M.I.), course, year, & section.
