B. PAY AT THE CASHIER THE PROCESSING FEE FOR COMPLETION OF INC GRADE. (As of now, online payments are not yet accepted at the cashier's office)

1. Fill-out the form below or click on this link.
2. Once you download the form, FILL-OUT ALL THE NEEDED DETAILS.

3. SCAN the O.R. and COMPLETION FORM and EMAIL them to the college secretary at evsucaad2012@gmail.com. Write in the subject line COMPLETION FORM, FULL NAME, COURSE YR & SECTION.
Example: COMPLETION FORM, DELA CRUZ, JUAN A., BSID 1-A
4. Send also a copy to your professor/ instructor as proof.
5. Once the college secretary receives them, he/she will forward the completion form to your professor/ instructor (who are faculty members of CAAD) for your grade.
6. Once your grade is written on the form, the college secretary will have it signed by the signatories.
7. Once your form is received at the Registrar's Office, your INC grade will be changed.
