All registrations must be completed online and a valid sports physical must on file before the first day of practice!


If your student did sports in 6th or 7th grade and you believe their physical is still valid, please leave a comment here (they are private) or email me at Active_PAC@evergreenps.org and I can look it up for you.  


Transportation update: Transportation will no longer be able to provide after school activity busses.  If your student does a sport or after school club you will be responsible for arranging a way home.

ACTIVITIES & ATHLETICS REGISTRATION


Step 1: Direct Website: https://wa-evergreen.intouchreceipting.com


Step 2: Choose the student you are registering


Step 3: Click on “Register for Athletics and Activities”


Step 4: Pick either 25-26 MS Intramural/Club or 25-26 MS Athletic Registration - Click on Manage (in blue), then click on Fill Out for each form. You will have four forms to fill out for sports, only one form for Intramural/Clubs, and you are REQUIRED to pay for ASB to participate.


➜Clubs & Intramurals is only 1 form to fill out. Clubs do not require a Sports Physical.


➜The required Athletic registration forms are:

● 25-26 Pacific Athletic & Activities Registration

● 25-26 Pacific Informed Consent

● 25-26 Pacific Transportation Acknowledgement

● 25-26 Pacific Consent to Share-Online Scouting


Step 5: Make sure to fill out ALL boxes on the forms, including a valid insurance number, and then click SUBMIT at the bottom for each form. If you missed a box on the form, you will not be able to submit the form. I will not approve applications without insurance numbers.


Step 6: Pay ASB Fee of $20 - do not let this stop you from sports/clubs! Please reach out to me if you need assistance. You may already have this waived if you completed the Benefits form.


Step 7: Bring a current Sports Physical to the Activities Office starting Monday August 11th, 8-3pm, or upload and email a copy to Active_PAC@evergreenps.org putting your students full name in the subject line.
