If you need help covering the required $20 ASB fee this information is for YOU!  Follow the steps below. You may skip Step 2 if you already receive any state benefits and ONLY do step 2!! 

Step 1: Complete online or print & return the 2025-26 Family Benefits Survey - This is required if you do not currently receive state/federal assistance benefits. 

Step 2: Complete the Consent to Share Form.  This is required for all families who are hoping to qualify for benefits.
