The six steps listed below are the best and only way to register as a new student!
Please complete all six steps below to ensure you appropriately register for Santa Teresa High School!

Please do not upload or email documents with your online registration. You must submit a copy of all documents to our Registrar in the Counseling Office.

Online Registration Summary (printed from Step 2; do not upload online)
Official Birth Certificate (government-issued; foreign must be translated)
Parent/Guardian ID (driver’s license or state ID)
Proof of Residency/Address (from Step 3)
Grade Printout or Parent Portal Screenshot (not official transcript/report card)
Immunization Record & TB Test Results
Santa Clara County residents: Previous negative TB record accepted; otherwise submit TB risk assessment form (signed/stamped by health provider).
Outside county: TB risk assessment form required.
Current IEP or 504 Plan (if applicable)
Income Eligibility Form (strongly encouraged for all 9th grade families; provides AP/college test fee waivers, internet discounts, and grant opportunities).
Additional Forms (if applicable)
Health Forms – submit only if required.
Migrant Education Program Form – for families engaged in agricultural or similar work.
CCGI Opt-Out Form – ESUHSD partners with the California College Guidance Initiative to give students free accounts to track academics and store college application documents. Participation is optional, but opting out requires a signed form. Opting out is not recommended due to program benefits.
Bring ALL COPIES of required documents to the Registrar's office at Santa Teresa High School located in the counseling office in room 109.
We are open from 8:30am - 3:00pm.
