If you are joining our LPDC and you already have all of your hours for renewal, we still require an IPDP on file. Please follow our guidelines for writing an IPDP - if you are transferring enough CEUs/contact hours to renew your license.
If you are joining our LPDC and you don't have all of your hours towards renewal, follow our guidelines for writing an IPDP - if you are drafting a new IPDP after joining our LPDC.
We MUST have an Educator Leaving an LPDC form on file before we can verify hours from previous districts

Non-Traditional Coursework Pre-Approval Form
Pre-approval must be obtained for non-traditional professional development where a certificate of attendance is not obtained
Educators-Exiting-an-LPDC-Form.pdfThis form is the only one that we can utilize. It must be completed accurately and have a valid signature from the educator's previous LPDC.
