We can give your child medication at school only when parents:
Bring the medicine in the original box, tube, or bottle.
Prescription medicine must have a label that includes the child’s full name, date of birth, name of the medication, name of prescriber, date, time, amount of dose, and how dosage is to be given i.e. oral, injection, etc.
Over-the-counter medicine will be given per directions on the container
No off-label doses will be given
Fill out and sign the Medication Sheet that is in the nurse’s office. This gives us permission to give your child the medicine.
Provide a handwritten, signed note giving permission to staff to apply any lotion, cream or ointments that don’t need a prescription for up to five days. After five days, a prescription is required.
