A written order from your child’s physician is required yearly for prescription and over-the-counter medications for pre-K-5. Ask your child's medical provider to complete the medication authorization form and return the form to the school nurse (see below).
Parents or guardians are required to fill out a medication authorization form to be turned in with the order from the medical provider.
All medication must be in the original container. Pharmacies will provide you with a second bottle to split the prescription if you ask. It's best to let them know before you arrive.
Parents/Guardians must assume responsibility for safely delivering medications to school. Students may not keep medications on their person/backpack or self-administer any medications (any product with an active ingredient) while at school, except for inhalers and epi-pens with a doctor's order.
