Medication Administration in School:
Epping Middle High School does provide over the counter medications such as Tylenol, Ibuprofen, Benadryl and Tums.
Prescription medications:
Any medication prescribed by your doctor must be brought in by an adult, in the original prescription bottle, along with a written physician order or a completed Administration of Medication form with the physician’s signature from your doctor. A parent/guardian signature is also required under the Parent/Guardian Permission For Giving Medication At School section on the Administration of Medication form. All medications must be brought to the Health Office and administered under the supervision of the nurse. The student should not possess or ingest prescription medication (other than inhalers or EpiPen's) or over the counter medications in a location other than the Health Office. If a student plans on self carrying his/her inhaler and/or EpiPen, a Permission to Self Carry/Self Administer Medication form must be completed and signed by a physician.
No over the counter medications will be administered to any student without parent/guardian permission on file in the Health Office. Medication selection and permission can be given by completing the medical forms section in PowerSchool (online) at the beginning of the year.
Special considerations:
Antibiotics: If possible antibiotics should be given at home. If there is a need to give a dose at school we recommend you ask the pharmacist to make you a small second bottle, labeled with just enough for the school dosages. No medication will be given to a student unless it is in the original bottle, with prescription label, accompanied by a written physician order and parent consent.
Emergency Information:
All parents/guardians are required to provide current emergency contact information for their child at the beginning of EVERY year by completing their information in the medical forms section of PowerSchool. This link is sent to parents by email prior to the beginning of the school year. The purpose of this is to assist the school in reaching parents or guardians as soon as possible should an emergency arise. Students who are 18 or older may update this form themselves. Parents are also required to complete a medical section. In this section, please indicate ALL health/medical conditions as this information does not transfer from year to year. Please notify the Health Office of any changes in pertinent information occurring during the school year. Please call the front office staff if you need assistance with this process.
Immunizations:
New Hampshire law HeP- 301.13-15 requires that all children enrolled in any school, pre-school, or child care have certain immunizations to protect them and those around them from certain vaccine preventable diseases.
Prior to school entry, all new students must present an immunization record or religious or medical exemption to the school nurse. Information on medical exemptions, and the form to be notarized, can be found on the NH DOE or can be obtained from the Health Office. Documentation of all student immunizations, as well as religious or medical exemptions, will be kept on file in the Health Office at your child's school. Please contact the school nurse for further information on exemptions. Failure to comply with immunization requirements will result in exclusion from school.
When your child receives immunizations, please provide an updated copy to the Health Office. The nurse will contact you if any additional immunizations are needed.
Injury or illness:
Any student who sustains an injury or becomes ill during school hours should go to the Health Office for evaluation. Often what appears to be a minor injury or illness is more serious; therefore, please permit the nurse the opportunity to evaluate the situation and make the appropriate recommendation for care. Students needing to be dismissed from school due to illness or injury should be dismissed through the Health Office. We ask that students do not call parents before coming to the Health Office if a student requests dismissal. The Health Office will contact the parent/guardian or other adult designated on the emergency form to obtain permission for dismissal.
