When it is necessary for a child to have medication during the school day, the school nurse must have the following information:

''Request for Administration of Medications'' form signed by parent/guardian. Click Here to download the medication form.
"Permission to Self-Carry Medication" form signed by parent/guardian if student will be in possession of medication and self administer during the school day. Click Here to download the medication form.
Medication in the original container if over-the-counter or in properly labeled prescription container.
Doctor's signature on the bottom of the ''Request for Medication'' form for all prescription medications, to confirm diagnosis and medication therapy while student is in school.
NOTE: The Health Office will fax the form to your doctor's office for their signature.
