欠席・遅刻・早退・アフタースクールケア・帰宅方法の変更は、すべてこちらのフォームからご連絡ください。

If the school needs to keep medication or an EpiPen, please ask your child’s doctor to complete the “Medication Administration Request Form” and bring it with the medication/EpiPen to the EIS Office.

学校で薬やエピペンを保管する場合は、上記フォーム'与薬依頼書'を医師に記入してもらい、薬・エピペンと一緒にEISオフィスへご提出ください。

If your child has any allergies, please provide a doctor’s note with detailed information. Please send it to the EIS Office.

お子様にアレルギーがある場合は、詳しい内容を記載した医師の診断書をオフィス（office@enishi.ac.jp）までご提出ください。
