When we head back to school in August, youโll be doing some fun activities based on the books you read. Summer can fly by, this form is a great way to jot down the important information so you don't forget a single detail!

Make it your own: You can fill out however many forms you would like (one for each book, or just one total).
Bring it back: If you do choose to fill them out, bring your completed forms with you on the first day of school!
