Waiver is the “Medicaid Home and Community Based Waiver Program”. Waivers pay for supports and services that eligible people need to live in the community. Apply quickly for a Waiver at your County Mental Health Office – because if you qualify and there are no funds – you are put on a waiting list.

Eligibility: eligible for ID Services, ICF/ ID eligibility, financial eligibility (you can lose funding if no longer meet criteria)

Two waivers: Person/ Family Directed Support (P/FDS) ($26,000 cap) and Consolidated Waiver (no cap, includes additional service of community living arrangements). There is also an Adult Autism Waiver (1-866-539-7689)

PUNS is the “Prioritization of Urgency of Needs for Services”. It gathers information to confirm how soon you will need services. You need PUNS if you or your family need services now you are not receiving or will need in the next five years.
