PLEASE FOLLOW THESE STEPS BELOW, ONCE YOU HAVE SCHEDULED AN APPOINTMENT THROUGH TELEHEALTH FOR A RENEWAL.
Fill out a Renewal Intake Form
Once you schedule an appointment, a Renewal Intake Form will be emailed to you. Please fill it out, as this information is necessary for your renewal.
Submit your renewal application through the state
Renewing your card is very similar to applying for your first card. You will use your same UtahID and EVS account.
You’ll receive an email about renewing your card 30 days before your card expires. You can renew your card before it expires, but you won’t get a new card until your current card expires.
Follow these steps to renew your card:
Go to evs.utah.gov and click the “Log in” button at the top right.
You will be taken to the UtahID log in page. Log in with your UtahID.
On your electronic verification system (EVS) account page, click the “Patient” button on the left hand side.
Click the “My application” button under the “My profile actions” heading.
Click on the application you need to renew under “Patient ID”. Your application status will change to “Active pending renewal” 30 days before your card expires.
Click the "Resubmit application" button on the left hand side.
7. Check your contact information and make sure it’s correct. Update any new information. Click the “Next” button on the right side of the screen.
Read the patient acknowledgement statement and click the “Acknowledge“ box.
Read the medical research statement. If you want to opt out of medical research, click the “Opt out of medical research” box. Then click the the “Submit for certification” button.
Click the “Submit for certification” button.
Your medical provider must enter a new certification for you after you’ve resubmitted your application. Contact your medical provider and set up an appointment to renew your medical cannabis certification.
You’ll get an email from us with instructions to pay your medical cannabis card fee after your provider submits your new medical cannabis certification in the EVS.
Once you have completed these steps, your QMP will be able to recommend that the state renews your card. You are ready for your appointment!
