Student Driving Forms

All student interns who are driving themselves and volunteers and supervisors who plan to drive their interns, must complete this form.   Those working for City of Elk Grove, EGUSD, and Cosumnes CSD are covered by their employers so under auto insurance, write, "covered by [employer's name]."  Please email completed form to Cyndi Muscat at CMuscat@egusd.net

Proof of updated Auto Insurance must be provided (unless you work for City of Elk Grove, EGUSD, or Cosumnes CSD).   Please send completed form to to Cyndi Muscat at CMuscat@egusd.net
