General Rules when applying for Illness or Misadventure
An Assessment Appeal Form must be collected from the Subject Head Teacher on the day of return from absence when a task or due date has been missed. (Note: A separate appeal form will be available for those students who do a TVET course from the VET Head Teacher and TAFE).
The appeal form must be fully completed and be accompanied by a medical certificate (in case of illness) or other supporting documentation (in case of misadventure).
The completed appeal form must be lodged with the Subject Head Teacher within two days of return to school after a task or the due date for a hand-in task has been missed, or where special consideration is sought.
Any appeal lodged after this time will NOT be accepted, except under special circumstances as determined by the Principal.
No application for appeal will be accepted after a task has been marked and returned.
Illness on the day of a task, during a task or prior to commencement of a task
If you are absent on the day of a task due to illness, you or your parent/carer is required to:
telephone the school to let the Deputy Principal Senior School (Examinations) or Subject Head Teacher (assessment tasks) know of your absence
obtain a medical certificate for the time you are away
on the day of return, collect an Assessment Appeal Form from the Subject Head Teacher
complete this form and promptly return to the Subject Head Teacher for recommendation and comment
if the assessment is a hand-in task, the task must be handed in before 8.55am on the day of return
Any student who misses an exam will be required to sit the exam within the exam time period. Arrangements are to be made with the Deputy Principal Senior School only. Missed exams will be rescheduled at the earliest time available in the examination timetable approved by the Deputy Principal Senior School or after the completion of the examination period at a time approved by the Subject Head Teacher.
Appeals lodged after two days of return to school will only be considered under special circumstances as determined by the Executive.
Under normal circumstances you will be expected to sit the missed assessment task as soon as possible after your return to school. This assessment will be reviewed by the teacher and given appropriate feedback.
In the case of serious illness or extended absence, especially where several tasks have been missed, new dates may be negotiated with the assistance of the Deputy Principal Senior School.
Medical Certificates
Medical certificates should cover the entire period of the student's absence, not simply the day of the task. Any subsequent medical certificates should (where possible) be from the same doctor or medical centre.
Doctor’s certificates must show:
date of appointment
your name
dates you are unfit to attend school and are expected to be ill
doctor’s name
address and phone number of medical surgery.
Where a student has a chronic medical condition, they will be required to obtain a medical certificate for each assessment task/period. One medical certificate at the commencement of the course is insufficient.
Illness and Misadventure at school before a task
If you become ill while at school before the commencement of an assessment task, you should:
inform the Deputy Principal Senior School (Examinations) or the Subject Head Teacher (Assessment Tasks).
collect an Assessment Appeal Form from the Subject Head Teacher.
obtain permission to go home if you are too ill to attempt the task. You must obtain a medical certificate from your doctor or Chemist to present with your appeal form.
on the day of your return, take the form immediately to the Subject Head Teacher for comment.
submit the completed form with the medical certificate within two days of your return to the Subject Head Teacher
Absence from a task with prior approved leave
If you know you will miss a task or the deadline of a hand-in task because you will be absent from school on approved leave, you are required to:
consult with the Subject Head Teacher involved in advance
negotiate a new date for the task. This will usually be before the date of the scheduled task
submit an Assessment Appeal form to the Deputy Principal Senior School (Examinations) or Subject Head Teacher (Assessment Tasks) in advance of your absence. You should supply all the necessary details, including a letter from your parent or guardian.
Note:
Approved leave will only be granted where the date of your absence has been set externally and cannot be re-negotiated. Examples of this leave include school representation (eg SRC or Sport) Overseas Exchange Programs, scheduled medical appointments including hospitalisation).
Under changes to the student leave process, Principals can no longer approve leave for family holidays. As a result it would be appreciated if families did not organise extended holidays during the Higher School Certificate Course.
Absence on the day of a task for reasons other than illness
If you are absent from a task because of misadventure or a personal matter you must follow the same procedures for absence due to illness.
Application for Illness or Misadventure. What happens when this form is submitted?
When you lodge your appeal, you are requesting consideration of your special circumstances.
Your application will be considered by the Assessment Task Committee.
Appeals for missed tasks must be lodged within two days of returning to school.
Appeals will only be considered beyond two days in extenuating circumstances
It is NOT automatic that your appeal will be accepted. You should contact the Deputy Principal Senior School if you have any queries regarding your appeal.
NB: A record of all students who have missed tasks will be kept by the school.
