Students who are absent due to illness need to complete the following google form within 48 hours of returning to school. This form is ONLY FOR ILLNESS. 

EHS Choir Rehearsal ILLNESS Absence Form

Student who are requesting being absent for any reason other than illness need to fill a Rehearsal Conflict Request Form. These are easily availible in the choir room and should be submitted to Mr. Lisner in the Rehearsal Conflict Request Form Dropbox located outside Mr. Lisner's Office. Only physical forms will be accepted, do not email them - they will not be accepted via email. 

Each student is permitted a maximum of two absences per concert project, including absences due to illness. All non-illness absences must be submitted for approval at least two weeks in advance using the below form and are subject to the director’s discretion. A separate form must be completed for each individual absence.
