Forms (A hard copy of these forms were sent home Friday, April 1st)

• Camp Foley Student Release Form

• Student Edina Public Schools Overnight Field Trip Authorization and Personal Health History Form

• Dietary Restrictions Form for students and adult chaperones

• Camp Foley Adult Release Form

• Adult Field Trip and Travel Chaperone Agreement

• Background Check Form The form must be completed in black ink. Send or drop off completed forms and your check for $15 to the district office. No one at Highlands can handle this for you. It usually takes 2-3 weeks for the processing so please turn in the forms to the District Office ASAP. If you completed the forms in the last 4 years, you do not need to do another.
