Medication Form- This form is for medication to be given at school for your child. Please fill this form out and return to the nurse with medication.
Asthma Action Plan - If your child is an asthmatic, this form provides us with guidelines on how to treat your child in the event they have an asthma flare up/attack while at school. Please take this to their physician to fill out and return to nurse.
Inhaler and Epinephrine Self Administration forms - Students are allowed to carry and self administer their own asthma inhaler or Epi-pen if these forms are completed by the doctor and parents. Have MD sign for consent before allowing child to carry inhaler or Epi-pen.
