Please complete and return the attached form to your school nurse by the end of the first week of school. Please be sure to complete both sides!
If your student takes medications and they need to be given while your student is in school, please contact your student's doctor to obtain an order for the 24-25 school year.
This order can be emailed to your school nurse.
Please complete the attached parental authorization form and return it to your school nurse.
If your student takes medication in a pill form, please be sure the medication is in its original container from the pharmacy and a second empty prescription bottle to send on field trips throughout the year.
All medications are to be picked up at the close of the school year or they must be discarded. You can coordinate a time to pick these medications up with Mrs. Formica
Health Screenings will be performed the week of October 28th-November 1st. These screenings will include height and weight, vision, hearing, and postural.
If you wish to opt your student out of height and weight, vision, hearing and postural screenings, please complete the attached Opt-Out Form!
If your student wears glasses or contacts, please be sure they bring them with them when screened for vision.
Per Massachusetts state regulations, all 4th-grade students are required to have a physical exam, which must be submitted to your school nurse. Your student's physician can fax or email the exam to your school nurse or complete the attached form for physical exams. If you have any questions, you can reach out to your school nurse.
During the first days of school, your child will learn about how we manage food allergies in the classroom/school; they will also learn about foods that classmates are allergic to. We are all unique, and we learn to care for each other at school.
There are children in your child’s grade with severe peanut/nut allergies. To reduce the risk of exposure, the classroom will be peanut/nut safe. Please do not send peanut or nut-containing products for your child to eat during snack time in the classroom. If your child has eaten peanut or nut products before coming to school, please be sure your child’s hands have been thoroughly washed before entering school.
Since lunch is eaten in the cafeteria, your child may bring peanut butter, peanut or nut products for lunch. In the cafeteria, the lunch tables are washed before students sit to eat, and students sit with enough distance between them to be safe eating a lunch that contains potential allergens. This plan will help to maintain safety in the classroom while allowing non-allergic classmates to enjoy peanut/nut products in a controlled environment. If a child does eat peanut or nut products for lunch we have a system in place for them to wash their hands when they are done eating.

Protect A Life...be a PAL
Choose 1 small snack to send with your child daily. Students continue to work at their desks while they eat snacks (no utensils needed, please). Your child should be able to eat their snack in about 5 minutes. An appropriate serving size is 1 cup. Some classroom snack suggestions include:
Fruits: grapes, apples, pear, dried fruit (raisins), banana Vegetables: carrots, celery, cucumbers, green peppers Cheese: string cheese, Babybell Cheese, sliced cheese Cereal: (no sugar added): Cheerios, Rice Chex, Corn Chex Other snacks: pretzels, whole grain crackers
We would prefer no: powdered snacks such as; Cheetos/Doritos, sticky foods, or candy
Your child should also bring a reusable water bottle they can refill at school as needed.
Parents, please pack an extra set of clothes to keep in your child’s backpack every day in case your child has an accident, gets wet at recess, etc.
