Please complete and return the attached form to your school nurse by the end of the first week of school. Please be sure to complete both sides!
Meadow Brook Elementary School will be conducting health screenings as follows:
Hearing and vision for all students in grades kindergarten-2nd grade will take place in late September/early October.
Vision screening will take place in late September/early October for all students in Pre-K.
BMI (height and weight) will occur at the end of January.
Pre-K Screenings for incoming students will occur in March.
If your student wears glasses, please be sure they have them when they are called to the nurse's office for vision screening!
Kindergarten Orientation Nights are scheduled to take place in March. You will receive a notification as we get closer.
Kindergarten screenings for incoming Kindergartners will occur at the beginning of May. You will also be notified as we approach these screenings as well.
This order can be emailed to your school nurse.
Please complete the attached parental authorization form and return it to your school nurse.
If your student takes medication in a pill form, please be sure the medication is in its original container from the pharmacy and a second empty prescription bottle to send on field trips throughout the year.
All medications are to be picked up at the close of the school year or they must be discarded. You can coordinate a time to pick these medications up with Mrs. Barbeau.
Medication forms will be mailed home at the end of May. Forms will be sent home in the backpacks of 2nd graders.
Per Massachusetts state regulation, all students in Kindergarten are required to have a physical exam and this needs to be submitted to your school nurse. Your student's physician can fax or email the exam to your school nurse or can complete the attached form for physical eaxms. If you have any questions you can reach out to your school nurse.
Choose 1 small snack to send with your child daily. Students continue to work at their desks while they eat snacks (no utensils needed, please). Your child should be able to eat their snack in about 5 minutes. An appropriate serving size is 1 cup. Some classroom snack suggestions include:
Fruits: grapes, apples, pear, dried fruit (raisins), banana Vegetables: carrots, celery, cucumbers, green peppers Cheese: string cheese, Babybell Cheese, sliced cheese Cereal: (no sugar added): Cheerios, Rice Chex, Corn Chex Other snacks: pretzels, whole grain crackers
We would prefer no: powdered snacks such as; Cheetos/Doritos, sticky foods, or candy
Your child should also bring a reusable water bottle they can refill at school as needed.
Parents, please pack an extra set of clothes to keep in your child’s backpack every day in case your child has an accident, gets wet at recess, etc.
