Transcript Request forms can be found in the School Counseling office (purple form found on the wall by the entrance)
Please Note:
Colleges must be up to date on Naviance before submitting your request
You may list up to 3 colleges per form
Submit a new form for each deadline/send date. One form should be used for multiple colleges if they share the same deadline or will be sent at the same time
All Requests must be submitted 10 days prior to your deadline
