Due to changes in IRS regulations effective this year, employers are no longer required to automatically mail Affordable Care Act (ACA) tax forms (Form 1095-C) to employees.
Employees may request a copy of their ACA form at any time.
To request a copy, please contact Bryar Kallhoff at bkallhoff@dwby.k12.mn.us. Requested forms will be provided within the timeframe required by the IRS.
Please note that you may not need this form to file your federal tax return; however, it is available upon request for your records.
