REQUIREMENTS:

•    Request Letter - duly signed and noted by the Immediate Supervisor and Division Chief, certifying that the proposed private practice will not affect the performance of the duties and responsibilities of the concerned DSWD personnel

•    Service Record

•    Certificate of Performance Rating (covering two rating periods for the year prior to the filing of the request)

•    Income Tax Return (For Renewal)

•    Other relevant supporting documents (e.g., contract between the applicant and employer, teaching loads and schedules, etc.)
