     c. Authenticated copy of Civil Service Eligibility report of rating and/or PRC Board/Bar Rating Certificate

 2.   Fully accomplished Personal Data Sheet (PDS) with recent passport-size picture (CS Form No. 212 Revised 2017) and Work Experience Sheet which can be downloaded here:

 3. Certificate/s of Employment from previous employer/s.

 4. Certificate/s of Trainings/Seminars

 5. Copy of Birth Certificate

 6. Copy of Marriage Certificate (if applicable)
