Times: 3:30pm to 5:30pm
All athletes must be signed in by 5:30pm
Guidelines:
This is for Winter athletes for various schools including Dallas.
All athletes MUST have a completed and signed PIAA Physical Form (Sections 1-6) to be eligible for a physical.
Vision will be checked, please bring glasses.
If you played a fall sport at Dallas and already had a physical for it, you only need the 1 page recertification form. Copies are available in the main office and below:

Please make every effort to be in compliance with this ASAP. Sports bags do not have to mesh or clear and must be dropped off into the bins as students enter the building each morning.

Please be advised that Dallas Middle School utilizes video surveillance to enhance security measures in and around the building. Video cameras are currently being utilized, video is recorded and stored, and is viewable by the principal and all Dallas School District administrators at all times.
