This survey is a requirement for all host sites as stated in your host site contracts. If you feel that there is no way you can complete this survey with your students, please let me know immediately.
All students who received nutrition lessons should take this survey.
In the beginning of the year, you may not know all of the students you will be teaching for sure. Best practice is to survey all students who have the potential to be taught by you. Use your best judgment. It is better to get a pre-survey in now vs it becoming too late and to not have a survey for them at all.
Please talk with your program director about individual circumstances if you have concerns.
INSTRUCTIONS:
1) Pre-surveys should be completed before you begin teaching any lessons and as early in the year as possible. As a general rule, all pre-surveys should be completed by November.
2) Post-surveys should be completed after all of your lessons (April/May timeframe) with them are completed and as late in the year as possible. All post-surveys should be completed by June 1. You can still teach summer school lessons after you complete the post-survey. These lessons are not included in our program evaluation.
3) As soon as you are able to at the beginning of the year, obtain a list of the students in each class you plan to teach in (or could potentially teach in). You do not need their full names. First names and last initials are fine.
4) Enter their names and classroom information into the Student ID Template share with you to create their 10-digit ID #s. Make sure to select the appropriate grade level in the ID template because this will complete the 10-digit ID. If you forget to put in the grade, their ID will only be 8 digits and will not work online. Note: I recommend bookmarking the ID template for easy access so you can use it to take student attendance throughout the year. You can also find it by searching "ID template" in your Google drive once it is shared with you.
5) Work with teachers to set up a time for the students to take the survey. For K-2nd grade students, I recommend working with the teacher to administer the survey together to make it easier.
6) Print the K-2 Survey Answers Worksheet and write your students' names on it. It may work best to split the class in two and give the teacher a worksheet with half of the student names and then you take a worksheet with the other half of the student names. This will make recording student answers more efficient.
7) Print out the Thumbs Up/Down Cards. You will need one set of thumbs up and thumbs down cards per student.
8) When you are ready to administer the survey, explain to the students that you are going to ask them a couple of questions that they get to answer with the thumbs up/down cards. Ask the students to not worry about how their friends answer. For each question, students should hold up one of their two cards.
9) As you or the teacher reads each question, mark down each student's answer on the worksheet by circling either the thumbs up or thumbs down picture. This is where it is very helpful to split the class up between two (or even three) answer recorders to make things run faster.
10) After you have collected the students' answers, you will have to enter them into the online form below using their 10-digit ID number instead of their name. Make sure you are using the correct link (either pre- or post-survey).
11) This process will be completed again later for the post-survey.

6) Determine the best way to give the students their ID # to put on the survey (ex. on name cards, stickers, pre-written on paper surveys, etc.). Please stress to the teachers/students how important it is to enter their ID number accurately on the survey. We often see students enter their school ID numbers instead of the 10-digit ID given to them.
7) Read the instructions on the survey before they begin. Students may ask questions while taking the survey. Note: 3rd grade students will likely need help reading the survey. I recommend taking the survey as a class by reading each question for them one by one.
8) If you choose to use the paper version of the survey, you will need to enter them one by one into the online survey form.
9) If you ever need to check to see if a specific class has completed the survey, email your program director.
Make sure to pay attention to which link you are sending (pre vs. post and specific grade level). Each survey offers a variety of links you can use. The shortened tinyurl link is the easiest; however, some organizations do not allow shortened links, so make sure to check with your school.

Each host site should complete the FSD Pre-Interview and Post-Interview with at least one FSD per year. If you work with more than one FSD, we would love if you were able to complete a couple of interviews. This information is very valuable to us!
This interview should be done in-person, via virtual meeting, or on the phone. We do not want you to just email your FSD the list of questions for them to complete.
After you ask each interview question, you should ask follow up questions based on their answer to gain more information as needed.
I recommend reaching out to your FSDs early to get something on the calendar as soon as possible for the pre-interview and then again in May for the post-interview so it can be completed by the June 1 deadline.
Once you have completed each interview, use the submission form below to type up their responses and submit to me. Please don't send me the written version because this has to be turned into our evaluators.
